To add to the list, illustrator -> photoshop.

-edit- either they used a filter or used some sort of 3D app then imported that into something like photoshop,psp, ect.

You can do that with photoshop using styles and filters

I used photoshop and Eyecandy 4000 plugin

if you are serious and you are doing a logo... you need to create in it Illustrator since it do it as a vector. That way it can be resized to anything and not loose any quality. You can always import it into Photoshop (my recommendation) for web use or certain print work. For banners, printing in high quality magazines then Illustrator is one to use.
