help with form

hi guys hope you can help me with this one.
I need to make a form with questions. the hard part is that after the questions has been answered, there has to be a rapport about the person who answered them. Its a psychologic test and afterwards there must be a bit of text that applies to the person judging from what he answered. I hope you can help me with this one.

Youl need some server side language for that (PHP, ASP...)
maybe its possible with Java as well...

I don't think Java is such a good idea unless it's JSP or Java Servlets. PHP is my recommendation.

Actualy you can just get the code by viewing the source of the page. The code starts where is says "<!--middle column-->"
