I'm a smoker and occasionally I have to spend a day or two getting the build-up nicotine stains off of my stuff. I did a search a while back and came up with a killer solution that literally melts nicontine build-up with very little ellbow grease and no super harsh chemicals. The solution:
1 gallon hot water
1/2 cup of 20 Mule Team Borax
1/2 teaspoon liquid dish detergent (Dawn cuts grease and would work well for the stains)
1 tablespoon ammonia
I like to use this in a spray bottle after the borax has all disolved. Have a bucket of clean water handy to rinse your rag out in because the stuff you melt off is uber-nasty and you don't want to contaminate your cleaning solution by rinsing your rag out in it.
I've also used that really harsh shower cleaner which works well, but this stuff works even better without the nasty fumes and burning skin.
